Do All Life Insurance Companies Offer No Medical Exam Options?
No, not yet, but we think it’s just a matter of time before they all do. The option to apply for life insurance coverage without having to go through a physical or medical exam is still relatively new. Since its introduction to the insurance industry, several companies have begun to jump on board, now offering it as a preferred method to apply for life insurance coverage. (For more information, read our “Life Insurance Underwriting: What to Expect When Applying“).
No exam life insurance will differ from company to company. This is not just by price but also on how it is underwritten, how much death benefit can be obtained, the age limitations on who can apply and how quickly an approval can be made.
Types of No Medical Exam Life Insurance
No medical exam life insurance is available in just about every form of life insurance. Whether you’re looking for temporary coverage or permanent coverage, you will likely be able to get it without having to take a medical exam.
Outside of underwriting, which will be touched upon shortly, the differences between a no medical exam policy and a policy that requires a medical exam are minor. For example, no medical exam life insurance will generally have a cap on the total death benefit that can be applied for as well as have maximum age limitations.
The death benefit cap for no medical exam coverage is often between $500,000 to $1,000,000 with most companies. The typical age cap for applying for no medical exam life insurance tends to be capped at age 60 to 65.
Life insurance that requires full underwriting will offer a bit more flexibility on coverage amounts and the age limits when applying for coverage. But, as you can see, the caps that have been set for no medical exam life insurance are very generous.
Caps and age limitations are going to vary not only by the company but also on the type of coverage that is being applied for. Below are the five different types of no medical exam life insurance coverage along with the standard guidelines when applying for coverage.
No Medical Exam Term Life Insurance
No medical exam term life insurance is the most popular of all the types of life insurance coverage. Term insurance is available in contract lengths of 10, 15, 20, 25, 30, and even 40 years. During the contract length that has been chosen, payments will remain fixed until the contract expires.
No medical exam term life insurance is often available to applicants that are between the ages of 18-65 and require less than $1,000,000 in coverage.
No Medical Exam Universal Life Insurance
No medical exam universal life insurance is available in both current assumption and no-lapse guaranteed options. Both options offer permanent life insurance coverage with minor differences.
- Current Assumption: Death Benefit & Cash Value Focused
- No-Lapse Guaranteed: Death Benefit & Low-Cost Premium Focused
Both no medical exam universal life insurance options are available to applicants that are between the ages of 18-75 and require less than $1,000,000.
No Medical Exam Indexed Universal Life Insurance
No medical exam indexed universal life insurance provides permanent life insurance with a focus on cash value growth. It is a newer form of universal life insurance coverage that utilizes the performance of popular indexes to determine an interest rate for potential cash value growth.
Most no medical exam indexed universal life insurance policies are available to applicants between the ages of 18 up to age 80. The typical cap on the death benefit is up to $1,000,000 with a few offering no cap as long as the amount is financially justified.
No Medical Exam Whole Life Insurance
Whole life insurance is one of the oldest forms of permanent life insurance coverage. A whole life insurance policy provides a locked-in premium along with guaranteed cash value growth with the potential to earn dividends. No medical exam whole life insurance options are available to applicants ages 18-65 that need life insurance coverage no greater than $1,000,000.
No Medical Exam Final Expense Whole Life Insurance
Final expense life insurance also referred to as burial insurance, is a second form of whole life insurance. This type of life insurance will not require an exam and is specifically designed to offer life insurance coverage in smaller amounts to seniors. The available age range for coverage is between 50-89 with coverage amounts starting at $1,000 up to $50,000 with no medical exam required.

How Does Traditional Underwriting with an Exam Work?
If you’re looking for the absolute best rates for life insurance, don’t be surprised if you’re asked to take a medical exam. There are still many life insurance companies that require a traditional underwriting process to qualify for their top rates. To be eligible for those potential rates, you will need to complete a necessary medical exam.
Underwriting begins soon after an application has been submitted to the life insurance company. Underwriters calculate your risk to the insurance company and assign you a health risk classification. The insurance company offers a premium based on the health risk classification you have qualified for as a result of underwriting.
If you are a higher risk, you will often receive a higher premium for your coverage. Those in good health with no existing medical issues will often have less of a chance of dying before the end of the policy, resulting in lower premiums. A fully underwritten policy requires a comprehensive look at your medical and health background.
To analyze your risk, the underwriter examines the following:
- Medical Exam – An exam provided by the insurance company in which a licensed examiner visits with you to collect blood pressure readings, blood sample, urine sample, and height & weight check.
- Medical Questionnaire – The application for coverage will consist of questions regarding your medical history. Some life insurance companies will require more information than others.
- MIB Search – MIB or Medical Information Bureau is a cooperative data exchange formed by the North American life insurance industry in 1909. MIB operates a database of medical information on individuals who have applied for health insurance, life insurance, and other types of insurance coverage.
- MVR Search – MVR or Motor Vehicle Report – During the underwriting process, the underwriter will check your MVR for significant violations. Major violations would consist of excessive moving violations, DUIs, suspended licenses, and reckless driving.
- Rx Check (Prescription Report) – Similar to the MIB, the Rx check or prescription check is a database of a list of pharmacy prescribed prescriptions that you have taken.
- Attending Physicians Statement (APS) – This is another name for your medical records. Depending on what your results return, the underwriter may need further information to complete their review. If this happens, the underwriter may reach out to your primary physician to review a copy of your medical records. This is not always a part of the underwriting process.
The underwriting process starts with the information you provide you provide on the application. An application often includes a health questionnaire for the initial evaluation. It may consist of necessary details, such as your weight, height, and medical history.
To verify your answers, underwriters may require a medical exam. Underwriters also use the exam to look for conditions that lower your mortality. These exams typically take about 30-45 minutes. For your convenience, many insurance companies can arrange for at-home or in-office exams.
The exam also tends to include health questions, height and weight check, blood pressure readings and blood/urine screening. The urine and blood sample tests for various health conditions potentially not discovered during a routine exam, such as tobacco use, cholesterol, diabetes, HIV/AIDS, or the use of recreational drugs.
The medical exam is a standard part of the traditional underwriting process, but it is just one component. Underwriters may also research your prescription history and request an attending physician statement (APS) to help aid in underwriting a life insurance application.
The APS which is also known as medical records provides insurance companies with an overview of your health, including the presence of any chronic medical conditions. Issues such as diabetes, cancer, asthma, sleep apnea, and dementia may impact your premiums as they are often classified as high-risk.
Insurance companies may also review your driving record. Your motor vehicle report (MVR) includes any documented violations, including traffic citations, DUI arrests, or accident reports. It covers a period of up to five or seven years.
After reviewing these details, the underwriter assigns your insurance classification and uses an actuarial table to determine your risk to the insurer.
The fully underwritten process may take two to eight weeks. The amount of time depends on the insurance company, the type of policy, and how quickly the underwriter receives the required documents.
With no medical exam policies, the underwriting process moves much quicker. You may receive coverage in a matter of days or even within the same day of applying.
How Are No Medical Exam Policies Underwritten?
Life insurance companies that offer the option to apply without a physical or medical exam will underwrite applications pretty similar to those that require an exam. Several of the same underwriting tools are utilized to underwrite the life insurance application and verify responses to personal and health-related questions.
The significant difference is that when you apply for a no medical exam life insurance policy, you are skipping the medical exam portion. This means that you will not be required to meet with an examiner to do any lab testing. This can cut down the underwriting time by at least one-week or longer.
With the advancements in technology, life insurance companies can utilize several underwriting tools to help assist in underwriting an application. For example, let’s say you’re being treated for high blood pressure or even diabetes. A simple check of any prescribed medications is going to reveal that you are being treated for one or more of those conditions. It is even going to tell the underwriter when the medication was prescribed as well as the last time the medication was refilled.
The life insurance company didn’t even need an exam to provide them this information. And if the underwriter needs additional information, they can easily order a copy of your medical records and review prior lab results and doctor notes.
Underwriting a no exam application will rely heavily on the use of the other underwriting tools such as:
- Telephone Interview
- Medical Questionnaire
- Medical Information Bureau (MIB)
- Prescription Check (Rx)
- Motor Vehicle Report (MVR)
- Attending Physicians Statement (APS)
- Fair Credit Reporting Act (FCRA)
Can You Get Good Rates With No Exam Life Insurance Policies?
Yes, you absolutely can.
We have spent some time discussing the different types of no medical life insurance coverage as well as the underwriting process for both life insurance with an exam and without an exam. As mentioned, underwriting is the main factor when it comes to determining your health risk classification. Your health risk classification is what determines your life insurance rates.
The primary health risk classifications include:
- Preferred Plus
- Preferred
- Standard Plus
- Standard
- Preferred Tobacco
- Standard Tobacco
- Substandard
The best health classification begins with preferred plus and will result in the lowest premiums. The substandard category offers the highest premiums and is typically reserved for those with higher risk health conditions.
When no medical exam life insurance first came out, the best available health class was standard or substandard. Because of this, the rates for a no medical exam life insurance policy were not nearly as competitive to a life insurance policy that required a medical exam. It did, however, offer convenience to those looking for fast coverage without the need of having to take an exam. This, in itself, was worth the extra cost to most people.
However, over time, several companies have started to include the preferred health class as an option for qualifying healthy applicants. With these new improvements, rates for no medical exam life insurance coverage are right on par with coverage that requires a medical exam.

Types of No Medical Exam Underwriting
Nowadays you can find several life insurance companies offering no medical exam options for both temporary and permanent life insurance coverage. This includes coverage options such as term life insurance, universal life insurance, whole life insurance, and final expense life insurance.
However, there are a few differences between the different types of available no medical options and how they are processed and underwritten. The four types of underwriting associated with no medical exam policies include:
- Accelerated Underwriting
- Simplified Issue Underwriting
- Graded Benefit Underwriting
- Guaranteed Acceptance Underwriting
Accelerated Underwriting
Accelerated underwriting or (AUW), for short, is the newest form of no medical exam coverage, and it is designed to offer very fast life insurance approval. As with the other types of no medical exam life insurance policies, AUW does not require an exam or blood/urine tests. Qualifying for an accelerated acceptance life insurance policy can provide the best rates for coverage as well as offer the quickest approval times.
For many insurers, AUW offers a suitable middle ground between simplified issue underwriting and fully underwritten policies. With accelerated underwriting, applicants may qualify for the preferred plus and preferred health classification without a medical exam. This makes AUW an efficient alternative to fully underwritten policies, especially if you are healthy and can meet the guidelines of the program.
The process typically begins with a quick and straightforward online application ticket. Once the company receives the ticket, a telephone application interview is conducted. The interview usually lasts 30-45 minutes and consists of both personal and health-related questions. Once the telephone interview has been completed, the underwriter will review the responses to the interview questions along with the background checks to provide a possible approval for coverage.
Who can qualify for accelerated underwriting?
Accelerated underwriting is designed to offer the fastest method to get quick, affordable life insurance coverage. For it to work, you must be able to meet the accelerated underwriting guidelines.
For an insurance company to offer this type of program, they need to significantly reduce risk and limit who will be eligible to apply. It is often limited to not only a specific age group (18-60) but only offered to the healthiest applicants. By doing this, it allows for the medical exam to be waived as well as the need for any potential medical records which can hang up underwriting times.
NOTE: In some cases, even the healthiest applicant may not qualify for accelerated underwriting. 50-60% of applicants who apply and meet the program guidelines will generally be eligible for accelerated underwriting. If you do not meet the accelerated underwriting guidelines, the insurance company will in most cases provide the option to continue underwriting if you are willing to take a medical exam. A second option would to having to take a medical exam would be to consider a simplified issue policy.
Simplified Issue Underwriting
Simplified issue underwriting was the first of the no medical exam options to be offered on a life insurance policy. These policies are perfect for people who may have minor health issues that would typically prevent them from qualifying for under an accelerated underwriting program.
Since simplified underwriting is more acceptant to specific health conditions, underwriting tends to be a bit longer underwriting process, especially if medical records are needed to help assist with underwriting. Applicants with no adverse health may qualify within a few days from applying.
Who can qualify for simplified issue underwriting?
Simplified issue underwriting is often available to applicants that are between the ages of 18-65. Coverage is limited to $25,000 up to $1,000,000. When the simplified issue was first introduced to the insurance market it was limited to a health risk class of standard. Now, it is widespread to find several companies offering preferred rates to qualifying applicants.
Simplified issue is an excellent alternative to accelerated underwriting and is a superior alternative to going through a fully underwritten policy. This option is very lenient to many health issues, and the preferred risk class is not off the table. Just be prepared that underwriting results can take a little longer.

Graded Benefit Underwriting
Graded death benefit underwriting is commonly associated with the type of life insurance called final expense or burial insurance policies. These are small whole life insurance policies that are purchased mainly for covering funeral costs and any other small remaining debts.
These plans can generally offer coverage starting at $2,000 and go as high as $50,000. And since these are whole life insurance plans, there is a guaranteed cash value account that will grow over time.
The application process is underwritten using the same guidelines of the simplified issue but is much more lenient on accepting applicants with riskier health conditions.
The only downside to this coverage is the graded death benefit part. Because this coverage will often accept medical conditions that are considered to be high risk, there is a two-year waiting period before 100% of the death benefit is payable. This means that if you pass away in policy years one or two, the only payable benefit is a full return of premium payments plus interest of generally 10%.
After the second policy year, the full death benefit is payable.
Who can qualify for graded death benefit underwriting?
As mentioned, a graded death benefit can be a good option for those who are seniors with smaller coverage needs and have riskier health conditions.
Guaranteed Acceptance Underwriting
As the name suggests, guaranteed acceptance underwriting guarantees coverage, no matter how poor your health may be. These policies do not require a medical exam or any health questions at all.
The underwriting process for guaranteed acceptance policies is often rapid. Many policies are approved within 24 to 48 hours. Most guaranteed acceptance plans are whole life policies that will provide coverage for life and benefit that will be paid to your beneficiaries after your death.
Guaranteed acceptance underwriting also comes with a few limitations:
- Lower coverage amounts
- 2-year graded death benefit
- Expensive premiums
Some companies may offer higher coverage amounts, but the average is between $1,000 and $25,000. This form of life insurance is also graded death benefit coverage, so there is a two-year waiting period before the full death benefit is payable.
The main drawback is the cost. Guaranteed acceptance is the most expensive type of no medical exam life insurance. However, it comes with the most straightforward underwriting process, which is mostly due to it having no health questions asked.
Who can qualify for guaranteed acceptance underwriting?
Just about anyone.
The only potential limitation would be not meeting the age guidelines. With most companies that offer a guaranteed acceptance life insurance option, the age limits are often between 50-85. You cannot be turned down for health issues or lifestyle habits. This can be a good option for those that cannot qualify for any of the other life insurance policies.

It is one of the very few companies to offer no exam underwriting to applicants over the age of 65, which tends to be the maximum age with most companies. How to apply for Pacific Life Smooth Sailing no medical exam underwriting
Smooth Sailing underwriting is only available to applicants that are between the ages of 50-69. Coverage up to $1,000,000 can be purchased, and the application process can be done electronically online or by paper submission. A medical exam is not traditionally required, but you must have had a physical with lab work done within the last 18 months from applying.
Since Smooth Sailing is a niche underwriting especially for age 50-69, it is ideal for seniors that still have a high need for larger coverage amounts. Underwriting is also very lenient towards many non-severe health risks and can be approved at very affordable rates.
Underwriting: Medical Information Bureau (MIB), Motor Vehicle Report (MVR), Prescription Drug Database, Public Data (Financial, Credit, Court Records, and Property Records)
Medical Records: Yes
Total Underwriting Time: 7 – 14 Business Days
Disqualifying Conditions Include: The following is a list of common factors that will disqualify you from the Pacific Life Smooth Sailing underwriting program.

Ages 20-55 - Included Riders: Accelerated Death Benefit Rider (Terminal Illness Only)
- Optional Riders: Children Term, Conversion Extension, Waiver of Premium
Highlights: In terms of actual coverage highlights, Principal doesn’t offer too much to get excited about, and this shouldn’t be taken as a bad thing. The term insurance still provides valuable death benefit protection, but the optional policy riders are average riders. The one unique policy rider is the Conversion Extension, which extends the amount of time in which the term insurance can be converted into permanent coverage. Where Principal does excel at is within their competitive sweet spots for applicants ages 35-55, and coverage amounts greater than $1,000,000. If you fall within these two categories, there is a good chance that Principal will be able to offer some affordable no medical exam rates with their term insurance.
Permanent Insurance
- Policy Name: Universal Life Protector IV
- Type of Policy: Universal Life Insurance
- Issue Ages: 18-60
- Contract Length: Permanent
- Guaranteed Interest Rate: 2%
- Minimum Coverage Amount: $50,000
- Maximum Coverage Amount: $1,000,000
- Included Riders: Accelerated Benefits Rider (Terminal Illness Only), Cost of Living Increase Rider, Extended Coverage Rider, Lapse Protection Rider
- Optional Riders: Children Term Insurance Rider, Salary Increase Rider, Waiver of Monthly Policy Charge Rider
Highlights: Protector IV offers a few more policy riders than their term insurance, but again, nothing too exciting. A free-rider worth mentioning is the Cost of Living Rider that will allow for additional coverage to be purchased if there has been a rise in the cost of living. However, electing this option will result in having to pay extra premiums. If policy riders are not a concern and the main focus is on affordable permanent life insurance protection, Principal will be most competitive ages 55-70 with life insurance needs that are at least $1,000,000 or greater.

Prudential can often offer these applicants that fall within those guidelines with affordable term life insurance rates. A really neat feature that comes included with Term Essential is the Living Needs Benefit Rider.
